That works great! I love the "referral" idea, and overall, little things like that make it very user friendly. I'm just interested - how customizable is this form? Can we add new fields at will?

thanks for testing out the form and I am glad you like it.

i have made it easy to customize the script itself (e.g. add header / footer html etc) but no, the fields are not easily added or removed; simply cause each requires some sort of validation process.

but i suppose if you know what you're doing with PHP, it would be easy to add fields and such...

i just added my own header / footer html since you used the form - so it will be very different from what you saw earlier (presentation wise).
