Why doesnt my form work correctly?

Good day,

I created a website that contains a form using FrontPage 2000 (Win98 PIII 600mhz system) that I published via FP2000 a few months ago & it worked perfectly. I just recently updated it with FrontPage 2000 and my new WinXP P4 1.6mhz system and uploaded it to the server via WS FTP and when you surf to the site and click on the submit form I get:

FrontPage Run-Time Component Page
You have submitted a form or followed a link to a page that requires a web server and the FrontPage Server Extensions to function properly.

This form or other FrontPage component will work correctly if you publish this web to a web server that has the FrontPage Server Extensions installed.

Click the <Back> arrow to return to the previous page.

I've gone through the forum before I posted but Im a bit stumped. I posted my website on 2 different url's and they both give me the same reply, the FrontPage Run-Time Component Page error message. On 1 of the sites (http://uniteddistributorscanada.hypermart.net) I uninstalled the FP extensions & reinstalled them back & it didnt work.
I then downgraded from FP2002 extensions to FP2000 extensions & that didnt work. Could this be a problem with my file or my servers? If you need to see my actual frontpage2000 file please feel free to ask me & I will post it for you. Could someone please look at my site (feedback section) and give any suggestions. The 2 url locations are:

http://uniteddistributorscanada.hypermart.net/

or

http://www.uniteddistributors.ca/

Thank you kindly

PS: my apologize if this is the wrong section to post in, I"think" its the right one.
